Our model

Prevention • Reset • Reintegration

One model sits behind everything we do. It gives every young person, family, school and commissioner a clear picture of where we step in, what we do and what we are working towards.

Stage 01

Prevention

Stepping in early, before concerns escalate.

The earlier we reach a young person, the more we can change. Prevention work focuses on young people who are showing the first signs that something is not right, so that concerns are dealt with before they become a crisis.

Who it’s for

  • Young people showing early changes in behaviour, mood or engagement
  • Young people whose attendance or behaviour in school is starting to slip
  • Young people at risk of exploitation or of being drawn into unsafe situations
  • Young people who are struggling emotionally and need support to cope

What we focus on

  • Understanding emotions, behaviour and the choices that follow
  • Recognising risk, including grooming and exploitation
  • Building resilience, self-worth and confidence
  • Healthy relationships and positive decision-making

What we’re aiming for

  • Concerns identified and addressed early
  • Young people better equipped to recognise risk and make safer choices
  • Continued engagement in school and positive activities
Stage 02

Reset

Interrupting the pattern and changing direction.

When things have already gone off track, a young person needs more than advice. Reset is focused intervention that interrupts the pattern, gets to what is driving it and replaces it with something that works.

Who it’s for

  • Young people whose behaviour is escalating at home, in school or in the community
  • Young people affected by trauma, or with significant emotional wellbeing needs
  • Young people at risk of exclusion, or already involved in risky or harmful situations
  • Young people where safeguarding or exploitation concerns have been identified

What we focus on

  • Understanding what sits behind the behaviour, using a trauma-informed approach
  • Emotional regulation and managing conflict
  • Breaking negative cycles and building new routines and responses
  • Rebuilding relationships with family, school and peers

What we’re aiming for

  • A reduction in the behaviours and risks that led to the referral
  • Young people with practical strategies they actually use
  • A clear, agreed plan for the next step
Stage 03

Reintegration

Supporting the return to education, family and community.

Change only lasts if a young person has somewhere to go next. Reintegration supports young people back into education, positive activities and relationships, and makes sure the people around them are ready to keep that progress going.

Who it’s for

  • Young people who have been excluded, or who are out of or disengaged from education
  • Young people returning to mainstream school or moving to a new setting
  • Young people rebuilding relationships at home or in the community
  • Young people finishing a period of Reset intervention

What we focus on

  • Planning the return to school or education together with the setting and family
  • Confidence, routine and readiness to learn
  • Reconnecting with positive activities and positive peer groups
  • Handing over clearly to the school, family and professionals involved

What we’re aiming for

  • Successful return to and engagement in education
  • Stronger relationships at home and in school
  • Progress that continues after our intervention ends

Moving through the model

Starting where the young person is

Not every young person starts at Prevention. When we receive a referral we look at what is happening and agree which stage is the right starting point. A young person can then move through the model as things change.

Assess

We understand the young person, the concerns and what is already in place around them.

Agree goals

We agree clear goals with the young person, their family and the referrer.

Review & report

We review progress throughout and report back on the outcomes achieved.
