Your route into Youth Services

There are two ways to refer a young person to us. Choose the one that fits you and complete the form. It only takes a few minutes.

Self / Parent-Carer Referral

For young people referring themselves, and for parents and carers referring their child.

  • No professional needed to refer
  • Tell us what’s happening in your own words
  • We’ll contact you to talk it through

School / Professional Referral

For schools, local authority teams, social care, health and other services.

  • Includes risk and safeguarding information
  • Suggest the stage of our model you think fits
  • Consent from parent or carer required

Referral forms are not monitored for emergencies

  • Immediate danger: Call 999 or go to A&E.
  • Urgent mental health help, where there is no immediate danger: Call NHS 111 and select the mental health option.
  • Non-emergency police matters: Call 101.
